FALL 2021 Entry Testing Requirement

FALL 2021 ENTRY TESTING REQUIREMENT

All students coming to campus for any reason must complete and submit documentation of entry testing for COVID-19. This could include coming to campus for classes but also for using the library, rec center, on campus events, etc.

If you have not completed testing yet, do so as soon as possible and submit results by October 15th, 2021 to avoid the $250 fine.

Test Requirements

  • All COVID-19 test documentation must include your name, date of birth, date of test and test result
  • Can test with either a PCR or antigen test
  • Upload test results to COVID-19 Test Results submission and select "Fall Quarter 2021 Arrival Testing - Late Submission (Students)"

  • Online-only students who will not be physically on-campus at all this quarter can request an off-campus exemption from surveillance testing at the same site as COVID-19 Test Results submission
