Reporting Last Term Form

(Formerly known as "Last Quarter Statement Form")

Students completing their degree need to report their last term so that we can update the SEVIS immigration database. If we do not receive and accurately record your last quarter/semester into SEVIS, you risk complications within the database. Our office is not automatically informed by the university when you have applied to graduate, so you need to inform our team separately of your plans so we can best assist.

Reporting Last Term Form

Please fill out this Reporting Last Term Form at the beginning of your last quarter/semester, whether or not you intend to remain inside the U.S. after you graduate. If you intend to apply for Optional Practical Training (OPT), you must also have this form signed by your academic advisor so we can ensure you are on track to graduate. After completing this form, please submit to your immigration advisor directly, or email it to isc@seattleu.edu
