LiVE is a wellness program designed to reward you for taking the small steps that lead to healthy habits. Because the healthier you are, the more you can enjoy life and do your best. When you participate in LiVE, you can earn points for chances to win exciting prizes including REI and Amazon.com gift cards!
LiVE participation starts with a well-being self-assessment that points out areas of strength and suggested areas to focus on in creating a personal health and well-being plan. Earn 1,000 or more points by December 17, 2017 and enroll in the 2018 medical plan at the lowest available premium. 500 points are automatically awarded when you complete the first step which is an online assessment. Earn more points by completing quarterly well-being challenges, going for an annual checkup and reporting your key health numbers (cholesterol, blood pressure and others).
Faculty/staff who are newly hired on or after October 1, 2017 will be able to enroll in the lowest available premium medical plan for 2018 whether able to earn 1,000 wellness points by December 17, 2017.
Protecting your confidential participation in LiVE is a fundamental component of the program. Your individual Limeade self-assessment responses will never be shared with the University. Self-assessment responses will be made available to the University when presented in aggregate – that means not sharing any health or well-being data that may be attributable to a single individual. Such aggregated reports assist the University in creating program activities. University Human Resources benefits administrators may access status reports via a Limeade secured site. These reports list each participant’s program points or challenge enrollment. The benefits staff uses such reports for communications to individuals or to assist with enrollment issues.
Your personal health data, including information secured with Limeade, is protected by the federal Health Insurance Portability and Accountability Act (“HIPAA”) and by strict contractual agreement.
