Request a T4 Form

If your site requires a submission form, please create a document with the form questions and answers (if a radio button, drop-down menu or check-boxes are needed) and our ticket agents will set one up for you.

For changes to existing forms, please email web@seattleu.edu with the details.

TerminalFour forms should not:

  • Take financial/credit card information (use a service such as EventBrite instead)
  • Take confidential/highly private information (e.g., social security numbers)
