Students can request personal copies of their conduct records, request that the Office of the Dean of Students complete a form for another agency or school about their conduct records, or request to view records in advance of a hearing. For all requests, please note that we will not provide information about records over the phone.
To request personal copies of your student disciplinary records, please fill out this Waiver of Confidentiality and submit your form along with the supplemental information listed below to the Office of the Dean of Students in person in Student Center 140, by email, or by fax to 206-296-6063.

Please note that the records may include redactions to protect the confidential information of other students who may have been involved in an incident or hearing process. Requests for personal copies will be processed within 4-6 business days.
If you have a form from an institution or agency that needs to be completed by the Dean of Students and mailed directly, please fill out the form and mail it along with a stamped, addressed envelope to:

Please contact your Conduct Administrator at least 24 business hours before the scheduled hearing to arrange a time to view documents related to a hearing. You will be able to review the documents in an office area and take notes, but you will not be able to copy or keep the documents. You may request personal copies of the records by following the process above. You will also have access to relevant documents during the hearing.
Students may request personal copies of their disciplinary records by following the process above and can then share the records with whomever they wish. Parents/Guardians wishing to speak with a University administrator about a conduct case must be authorized to do so by the student*. The student must sign a form specifying the name of the parent/guardian with whom the University can speak about a record or records and when such authorization should expire. Please note that the University is only able to speak about the individual student's records and cannot discuss the records of other students who may have been involved in an incident.
*As part of a sanction, under circumstances permitted by FERPA, parent(s)/guardian(s) of a student may be notified by letter when a student under the age of 21 is in violation of the Alcohol & Drug Policy and/or is placed on Disciplinary Probation, any form of suspension, or dismissed from the University.
