Vaccine Registration for Students

On April 13, 2021, it was announced that COVID-19 vaccinations will be required for students attending Seattle University in Fall 2021. Please follow these steps to register your vaccines: 

  • Once you have received a single dose of a one-dose vaccine, or both doses of a two-dose vaccine, you must register your vaccination details using the form below. Submission of this form with adequate documentation fulfills the vaccination reporting requirement.
  • More information on the process for requesting exemptions will be shared with students in the coming weeks.

Vaccine Registration for Faculty and Staff

Faculty and staff are required to be fully vaccinated for COVID-19 prior to Fall 2021.

  • Once you have received your final vaccine dose, you must attest to your vaccination status by using the form below no later than Sept. 6, or Labor Day.
  • Information on exemptions available to faculty and staff will be shared by early July.
