From this page you can take a self-assessment for  Depression, Bipolar Disorder, Generalized Anxiety Disorder, Post-Traumatic Stress Disorder, Eating Disorders and Alcohol Abuse. You may take one or more of the assessments.   Each screening will take 4 to 5 minutes to complete.

Your screening score is informational, not diagnostic. It indicates whether your responses are consistent with Depression, Bipolar Disorder, Generalized Anxiety Disorder, Post-Traumatic Stress Disorder, Eating Disorders or Alcohol Abuse and where to call for help. It is not intended to be used as a substitute for a visit with a health care professional.

When you click on the button below, you will leave Seattle University's web page. The University does not manage or control web sites not owned by the University. A core value and objective of this screening is to preserve participants' anonymity.

The University cannot identify you through your responses. The only information that can be reported to the University is aggregate in nature and pertains to general information regarding the number of screenings that take place, the results of the screenings, and general demographic information.

If you have any questions or concerns about the privacy and security of the online screening tool, please review the privacy policy prior to beginning the screening.
