Change in Number of Credits Form
Use this form any change in the number of credits for which you had projected for the academic year.

Course Projection Calendar
Use this form to indicate the classes you will take each year. This form is used to determine financial aid awards and the planning of classes for the academic year by the faculty.

Declaration of Credits Form
Use this form to indicate the number of credits for which you are planning to register. This form needs to be completed and turned in (along with the Criteria-based Aid Form) for you to receive financial aid.

Criteria-Based Tuition Aid Grant Application
Use this form to apply for STM criteria-based tuition aid. The School of Theology and Ministry grants criteria-based aid to students who demonstrate financial need, academic excellence, special merit or service, diversity and longevity in the program. Please submit this form with a copy of your 1040.

Scholarship Application for Leaders of Multicultural Faith Communities
Scholarships are awards of up to $1,900 per quarter (based on 6 credits) or $7,600 per year for two years. SU has created these special scholarships to recognize candidates for graduate study who are members of under-represented populations.

STM/ICTS Tuition Aid Grant Application Form
