Academic Forms
Change in Number of Credits Form
Use this form any change in the number of credits for which you had projected for the academic year.
Course Projection Calendar Use this form to indicate the classes you will take each year. This form is used to determine financial aid awards and the planning of classes for the academic year by the faculty.
Declaration of Credits Form Use this form to indicate the number of credits for which you are planning to register. This form needs to be completed and turned in (along with the Criteria-based Aid Form) for you to receive financial aid.
