Vision Plan
Vision Service Plan (VSP)
is administered much like a Preferred Provider Organization. There is
a list of providers that have agreed to accept VSP's schedule of benefits.
If you choose lenses or frames that are more expensive than the scheduled
amount, you pay the difference. You can go to a qualified provider who
does not participate in VSP, but reimbursement is only available up to
VSP's scheduled amount and you may be required to submit your own claim.
The following chart shows the coverage provided by the vision
plan.

Any licensed provider. However, VSP participating providers accept
payment at the plan's allowed reimbursement levels and will submit
claims on your behalf. If you use a non-VSP provider, you may
be required to pay up front for services and to submit your own
claim to the insurance company. |
