Letters of Recommendation 1. Speak to your recommender directly. He or she may ask for specific information before agreeing to write your letter. Be prepared to explain why you've chosen him/her as a potential letter writer, including what qualities you hope he/she will cover in the letter. 2. Provide your letter writer with useful information. The letter of recommendation request form provides space for you to attach your chemistry resume and you personal statement from the application you are preparing. This will help your professor(s) write an appropriate recommendation. Be sure to provide your professor(s) with any additional forms he/she will need to fill out. 3. Fill out the FERPA Information Release form, located here. Turn it into the chemistry department. Your letters cannot be processed until the chemistry department has this form on file. Once you have spoken with your recommenders, they will ask you to fill out the letter of recommendation request form .
