Manage Departmental Copy Cards
Once logged into the online request form, you will see a list of active departmental copy cards linked to the displayed budget near the bottom of the screen.

- Lost or stolen cards can be replaced by clicking the "Reprint" button:
- There is a $15 reprinting fee for each card
- On the confirmation screen, you can use the "Notes" field to denote a name change for the card or to indicate where and to whom the card should be delivered
- Otherwise, leave it blank and click "Queue Request"
- Continue making other requests or click "Submit Requests"
Reprinting a card automatically deactivates the old one so it is unnecessary to submit a separate request to suspend the card.
- Look for the "Add Department Card" section at the bottom of the page:
- Enter a unique name for the card (i.e. "Admissions 2", "Tutoring Program", etc.)
- Click the button with the green plus
- The "Notes" field can be left blank
- Click "Queue Request"
- Continue making other requests or click "Submit Requests"
Please be advised that there is a limit of two dept copy cards per budget.
- For cards that are no longer needed, click the "Delete" button:
- The "Notes" field can be left blank
- Click "Queue Request"
- Continue making other requests or click "Submit Requests"
- If the card is in your possession, please destroy it
