Seattle University IRB Protocol Forms

 All IRB submissions should include consent form(s) and/or a parental permission and assent form (if subjects are under 18 years of age), and be signed by all parties involved (primary investigator, co-PIs, and faculty adviser, if applicable). Submissions must include copies of current (within 3 years) CITI certificates for all such parties (investigators and faculty adviser). NOTE: NIH certification is no longer accepted for human subjects training. All PIs and faculty advisers must complete the CITI certification to submit a protocol application. See our page on Human Subjects Training.

All student researchers submitting research proposals to the Institutional Review Board must be working with a faculty adviser to complete their IRB submission paperwork.

The following documents are read-only Word documents, so please download and save a copy in order to fill out the submission form and proper appendices (if relevant) correctly.

After reviewing the checklist and FAQ sheet for Exempt reviews, if you believe your study qualifies as exempt, please complete and submit the Application for Exemption from IRB Review. Exemption applications usually take no more than 2-3 business days to process and issue an exemption letter (unless it is determined that the study should be elevated to a higher level of review).

IRB submissions may be sent electronically to irb@seattleu.edu -- however, submissions will not be reviewed without signatures of all parties. Thus, if PIs submit a Word doc for the main application, they must scan in signed copies of the final pages and include those as PDFs.
