2013 Plan Costs
Medical contributions are based on your annual rate of pay as of November 1, 2012. Your payroll contribution amount will remain the same for the entire 2013 plan year, even if you have a salary adjustment after that date. Dental contributions to enroll dependents are determined by your coverage choice but not your pay rate.

NOTE: The medical rates shown for dependents includes the cost to cover yourself.
