SU Insurance Information
Seattle University bills a mandatory health insurance fee to all full-time undergraduate students. Students who are already insured may request to have this fee waived by completing the Acceptance/Waiver Form online at SU Online SU Online under the student life section. If you should have problems completing the online form then please contact the Student Health Center. Please see the student handbook (pg. 28) for a full description of the mandatory "tight waiver" undergraduate student health insurance policy.

Insurance Claim Form
Insurance reimbursement for any fees incurred for services is the responsibility of the student. If you are a student insured under the Undergraduate, Graduate, Part Time or Law student health insurance plan, you do NOT need to submit a claim form; simply send any receipts and medical bills with your name and medical Identification # found on your medical ID card to the Claims Administrator for processing. Refer to your Student Health Insurance brochure for information on the Claims Administrator.

If current undergraduate students do not have health insurance coverage of their own, coverage is provided under the Student Injury and Sickness Insurance Plan.
