NOTICE: All forms on this page are for endorsements under the old (pre-September 1, 2003) endorsement regulations. Endorsement forms on this page can only be used by teachers who already have a valid Washington State teacher's certificate.

Applicants applying for the MIT program can not use the endorsement forms on this page. MIT program applicants must go to Forms and Documents.

DEADLINE for Completion

For endorsements able to be approved under the Pre-September 1, 2003 endorsement standards

All requirements must be completed, all work done by no later than August 31st, 2005, for all endorsements to be approved under the Pre-September 1, 2003 endorsement standards.
