Post-school Survey
2008 Post-school Survey:
The 2008 Online Post-school Survey is available now! To access the online survey
please go to the survey login page:
https://www.seattleu.edu/ccts/Survey/. CCTS will ONLY accept
surveys submitted using the online system for the 2008 leavers. Paper/pencil surveys
will not be accepted. It is suggested that you start preparing for the 2008 Post-school
Survey activities now. The information and documents below are designed to assist
you in completing Parts 1-3 of the survey. Part 4, the telephone follow-up interview,
takes place between December 1, 2008 and January 31, 2009 and will be accessible
on December 1st.
IMPORTANT:All special
education leavers must be included. These are the 2008 graduates and all dropouts
(grades 9-12) leaving during the 2007-2008 school year. Note: 120 districts did
not report any dropouts for the 2007 leavers. As part of the online survey system
districts will be required to certify that all graduates and dropouts have been
included. Thank you for your attention to this requirement (State and Federal
Statutes require this data collection, specifically 20 U.S.C. 1416(a) (3)(B) of
IDEA and WAC 392-172A-07015 and 07020 of the Washington Administrative Code).
Logging In:
| - | You must be a registered user to access the online system. |
| - | If you are not a registered user, please contact your District Manager for the
online survey or your Special Education Director. |
| - | If your district has never used the online survey and you do not have a District
Manager, contact Lisa Scheib at CCTS (scheibl@seattleu.edu). |
| - | Your password has not changed if you were a registered user last year. If you
cannot remember your password please reset it on the log in page. |
| - | Once you log in you will be asked to agree to assurances regarding the confidentiality
of the survey data. You will not be able to continue unless you agree to these assurances. |
Completing the survey:
| - | Parts 1-3 (from the final IEP) must be submitted before beginning Part 4 (follow-up
interview). It is recommended you collect the information for Parts 1-3 in the spring. |
| - | Part 4 (follow-up interview) is conducted between December 1, 2008 and January 31,
2009. All surveys must be completed and submitted by February 1, 2009. Part 4 will
be accessible on December 1, 2008. |
2007 Post-school Data Report:
| - | The 2007 Post-school Data Report for your district is located under the Post-school
Data Reports tab (at the top of the page) of your district’s online survey homepage. |
| - | The paper/pencil template of the survey, the FAQs, and other helpful documents related
to the 2008 survey are located under the Resources tab (at the top of the page)
of your district’s online survey homepage. |
Helpful Documents:
Keeping track of leavers’ spreadsheet(Excel): This Excel
Spreadsheet is a suggested way for organizing the list of your 2008 leavers. Feel
free to modify (add or delete columns) the template to fit the needs of your district.
Most of the column headers contain comments; to read them hover over the red mark
in the cell with your cursor. If there are multiple people making the follow-up
phone calls in December 2008 or January 2009, it is recommended that the District
Manager give each person their own spreadsheet for the youth they will contact.
This way the contact information for each youth is contained in a centralized place.
2008 Demographic Form (Word): Use this Word Document to collect the most
up-to-date contact information for each youth you will be contacting for the 2008
Post-school Survey. Getting this information now will make it easier to successfully
contact the youth in December 2008 or January 2009.
