Data and Service Request Forms

Use the appropriate form below in order to request support from Advancement Services. Please provide enough time for Advancement staff to service your request. If you have any questions about using these forms, please contact REHelp@seattleu.edu.

• Confidentiality Agreement 
Confidentiality agreement that all Alumni and Donor database users must consent to. Non SU staff must consent to this agreement before viewing ANY information from our database.
 
• Online Event Registration Requests
  Contact Advancement Services by emailing REHelp@seattleu.edu about the possibility of using online event registration for your alumni and donor event.
   
• RE Event Record Request Form  
  Use this form when requesting event records set up in Raiser's Edge.
   
• RE Data Request  
  Use this online form to request mailing lists and reports from Raiser's Edge.
   
• Research Request Form
  Use to have prospect research performed on a specified constituent.
   
• Email Blast Request Form 
  Use this form to request email blasts to alumni and donors, choosing from one of our available templates or providing full HTML content.
