Study Abroad Course Application Development

Directors are encouraged to develop an application which will aid with participant selection and provide pertinent student information. The following items should be requested on applications: student's full name (first, middle, last) as it will appear/appears on the passport; home, school and email addresses; cell, school and home phone numbers; gender; date of birth; social security number; SU student ID number; current student status; anticipated year of graduation; major/s and minor/s.
